Forms to Complete Before Your Child Starts Nursery
Before your child starts nursery we will ask you to complete a form which asks for personal details about your child, family details, health considerations, dietary requirements, allergies and previous/shared childcare experiences. We also ask for contact details in case of emergency. There are two sides to this form and we request that you complete both sides and sign in both boxes. If there are any amendments to this form during your child’s time at nursery CAN YOU PLEASE INFORM THE OFFICE IMMEDIATELY, thank you.
Personal Details and Emergency Contact Form
We will also ask you to complete a form which is much more personal to your child. It asks what toys your child likes to play with, do they have any pets or brothers or sisters. This will enable staff to get to know your child better and help ease their transition into nursery.
You will be given a Data Protection Privacy Notice which explains the types of data held about your child, why that data is held and who it may be passed on to.

Etherley Lane Nursery School receives funding from the Local Authority. To enable us to claim this funding we need parents to complete a Declaration Form confirming that their child attends this nursery.
The school also receives additional funding (known as early years pupil premium) for pupils whose families are in receipt of the benefits below.

- Support from NASS (National Asylum Support Service) under part 6 of the Immigration and Asylum Act 1999
- Guarantee element of State Pension Credit
- Child Tax Credit (with no working tax credit) with an annual income of no more than £16,190

Children who are in receipt of Disability Living Allowance are also eligible for Disability Access Fund which is an annual lump sum paid to the child’s early years setting.
It is in all our interests that we bring in as much money as we can to support our school and ensure all of our children get the best possible education. To help your child and your school, we can do a very quick eligibility check for this additional funding via Durham County Council. In order to do this we require some simple information which we collect by asking you to complete the following form.
A letter explaining why photographs and images of children are taken in nursery, what these images are used for, and how they are safeguarded and disposed of will be given to parents. We ask that you please read this information and sign and return the form if you agree to give permission to video/photograph your child in school and events.
At Etherley Lane Nursery School we may occasionally take children out of nursery on educational visits and we need parental consent to do this. As we are a nursery we need only one consent form completing and this covers all trips during your child’s time at nursery. We ask you to complete this form before your child starts nursery but we will give plenty of notice if your child is invited on a visit, and you will still have the option to decline your child taking part in the educational visit.
Educational Visits Consent Form
Every term the school is required by the DfE to complete a census of the children who are attending the setting. Information required for this census includes the child’s ethnicity, language, country of birth and nationality. The form below is given to parents to capture this information.
Administering Medication in School
Staff will administer medication to children at Etherley Lane Nursery School if requested to do so by a parent. Any prescribed medication must have an un-tampered prescribed label attached, with the child’s name and dosage clearly visible. If you would like us to give your child any medication a please complete the form below and hand it in to the school office with the medication.
